PROFESSIONAL SUMMARY Director of Social Services/Activities with more than 17 years in resident care. Focused on team coordination and leadership to provide exceptional resident and family support. Under my guidance as Director of Activities at Glencroft Providence Place, my team created a new 1200 sq. ft. Activity Center where there had been no designated space for many years. As Director of Social Services at Windsor, I completed 120 annual psycho-social assessments supervised 7 staff members trained 5-7 Social Work BSW interns annually was head of Interdisciplinary Care Team trained all staff in dementia/Alzheimer's Disease issues and handled resident/family issues in the RCFE along with Executive Director. As Activity Director there, I planned and implemented all entertainment, activities, outings and wellness programs for residents in coordination with the Activity Director in Skilled and Activity Assistant in Assisted Living. I am a Certified Dementia Practitioner (CDP), Certified Activity Professional (CAP) and certified in the Alzheimer's Association CARES Program (Advanced). Approximately 5 years ago, Windsor became the first retirement community in California to receive a 5-star rating for its skilled nursing facility from the U.S. Dept. of Health Services (DHS), and we received statewide recognition for being the "Best Retirement Community in Southern California, " with a current occupancy rate of 99%. I think I made a significant contribution to our community achieving this status for the first time.
