Highlights:
I have honed my assessment skills. I have a good working knowledge of Medicare criteria for both skilled nursing and acute hospitalization. I have created and implemented an Alzheimer's support group. I have participated in the planning and implementation of a pilot program at Florida Hospital. I get my point across in a tactful, direct way and then I am able to discuss the others' point of view to come to an acceptable conclusion. I am not afraid of public speaking or writing my own reports. I recently wrote a cardiac case manager's handbook for which I did all the research. This I did under my own initiative. My manager did not even know about it until it was complete and then she loved it!
Work with doctors, nurses, physical therapists, familys, and patients to plan a safe discharge. I am an integral part of the interdisciplinary team.
I had a significant role in the design and implementation of a pilot program for the rehabilitation of long-term vent/trach patients.
