Differentiated Guardian Home Care in a highly competitive marketplace by focusing on evidence based care which is patient centered innovative and individualized. Used an educational and consultative sales approach to educate physicians on the advantages of Guardian Home Care's disease specific management programs such as CHF COPD Orthopedic Fall Prevention Diabetic and Wound Care. Responsible for facilitating the referral process. Responsible for the coordination of information to patients family members physicians SNF/hospital staff the home health agency and any other involved entities related to the continuing care of patients in their homes. Received referrals and advised patients family members staff and physicians about programs available for patient care needs. Assisted in providing home care staff with adequate information of home care plan prior to patient's order for service and equipment ordering equipment from vendors and communicated with the home health agency. Provided home health agency with complete referral information to ensure timely appropriate care and to enhance relationships between the agency patients families physicians and referral sources.
