The initial meeting with the member an assessment is generated and some direction established on how to best help the member help themselves as long as they are able and willing. A Members visitation frequency is based on member's overall health and functioning. As a rule a Care Manager I will have a face to face meeting with members on biweekly and weekly basis. Visited are designed to assess the member for his/her needs and wellbeing and prevent gaps in time that a member may need assistance. In role I will i.e arrange transportation and accompany if needed. I am currently managing members the Long Term In Home program. Case Manager 2/13/12 - 5/21/13 PEOPLE OF COLOR NETWORK As a Case Manager my primary responsibility initially was to meet with client engage, coordinate, develop and implement a plan of care. (ISP) The plan included goals and corresponding objectives. Some of goals that were established for the client were common to behavioral health recipients. i.e. housing, services and food initially. I work as mediator as well as an advocated for the clients. Coordinating services with health care providers, landlords, social security as well scheduled home visits in combination with ISP goals. Case Manager 3/5/07 - 6/13/2011 LA FRONTERA My role while at La Frontera began as an adult case manager and concluded with my working with young adults. My case manager duties were similar to that of my earlier and latter roles of working with member/client with disabilities. The challenge was working with young adults 18 to 20. With young adults I learn diplomacy. With young adults I got the parent was well. The goals similar, but tailored with a young adult in mind. The ISP was generated on almost all occasions with parents or representative present. The young adult program differed in many respects. The rules for holding young adults accountable were design to be different in that they had developed fewer coping skills/strategies and therefore follow through on the same expectation as that of adults were not as pronounced/aggressively pursued. Inpatient Social Services Worker 7/06 - 2/07 UNIVERSITY PHYSICIANS HOSPITAL AT KINO CAMPUS In my role as Inpatient Social Service Worker. My responsibilities included, but were not limited to: Discharge Planning which included: I assisted patient with outpatient appointments prior to their being discharged home. i.e. PCP appointments, Specialist appointments, etc. I arranged for durable medical equipment and in home that services that may have been needed in their transition from Hospital back home. I implemented a Service Plan with the goals that were generated from patient input/feedback primarily. I provided relevant information to physicians and other caregivers. I assisted patients and family members locate needed resources following discharge.
