Qualifications / Experience East Valley Family Services Differential Response Case Manager 07/2016 - 10/2016 * Complete assessments of families that are considered a P3 level by DCFS for appropriate services that consider their unique strengths, risks and individual needs. * Conducts in-depth interviews with clients, families, and other appropriate individuals. * Maintain and complete all paperwork within designated timeline. * Coordinates the referral and intake process for the program including information and referral for self-referrals/walk-ins, and other community referred clients. * Input client information in the UNITY system. * Assess clients utilizing the NCFAS-G and complete a Safety Assessment. * Develop an intensive service plan for all families * Provides case management services to families and/or youth * Conducts weekly contact with clients and bi-weekly with families unless otherwise instructed. * Conducts client intakes for DR department program.
