* Assist adolescents with chronic illnesses conditions and disabilities to increase their independence skills educational opportunities employability and quality of life in preparation for adulthood. * Assist families in coping and managing difficulties and challenges associated with caring for children with chronic illnesses conditions and disabilities. * Research and provide information about and assistance gaining public-aid social services and entitlements to the aforementioned families and adolescents. Conduct intakes interviews and ongoing skills-assessments to the aforementioned families and adolescents. * Provide therapeutic interventions to the aforementioned families and adolescents. * Log interactions between this social worker and clients clients' families service organizations and volunteers. * Consult with social workers and case managers on client details and needs. * Develop and supervise volunteers and mentors working with the aforementioned adolescents. * Attend meetings and trainings relating to program and mentor development therapeutic techniques and social services. * Produce bio-psycho-social histories of clients. * Produce and practice general and specific goals and tasks relating to independence-skills development and enhancement. Create modify and update comprehensive individualized transition plans Likert-scale evaluations and other data-tracking and records documentation for the aforementioned adolescents. * Attend weekly meetings and receive supervision for clinical hours towards licensure as a clinical social worker (LCSW).
