Case Manager

I am seeking a position in Case Management. I have worked with the full age spectrum of individual as a home care case manager for 21 years with state Medicaid waiver programs. For the last 9 years I have done case management telephonically from my home office. I am highly organized and efficient in time management. I have excellent written and oral communication skills and am able to communicate effectively with individuals and groups from a broad spectrum of socio-economic backgrounds. I am able to work independently seeking minimal supervisory direction as needed. I am able work as a member of a multi- disciplinary team as well as leading a team of multi- discipline individuals. I am computer literate with intermediate skills in Microsoft Word, Access, Excel and Outlook and have worked with a variety of private sector and government Internet technology systems. My well-rounded nursing background in acute and long-term care, industrial and home care has prepared me to move quickly, but thoughtfully, adapt to change and always be prepared for the unexpected.
