A position that coordinates the care and service patients and promoting communication and collaboration skills with the patients. And caregivers, physician and other members of the health care management team. As a health care professional I will assess, implement, coordinate, monitor and evaluate available services and resources to promote quality, cost effective to patient's needs and resources. As a Manager I will assist with the providers/vendors to promote improvement and maintenance of social well-being of the patients and coping skills of family members and caregivers. As a newly graduate in health care I am excited and determined to get to job done. Core Qualifications * Minimum of twenty plus year's health care experience, and has one year of Utilization/Case Management, Quality Management or Home Care Coordination experience. * Demonstrates oral and written communication skills * Demonstrates ability to develop and maintain working relationships with physicians and work collaboratively with health professionals at all levels to achieve established goals. * Demonstrated excellent facilitation skills. * Ability to plan and schedule tasks and to maintain control of own work flow. * Experienced computer skills, advanced computer skills in power points and word excel with spreadsheets. * Have great decision-making skills and independent judgment and critical thinking skills. * Has great organizational skills, customer service, communication, and interpersonal communication skills proven.
