Description
Partnered with patients Perform initial clinical chart reviews of all patients to determine medical necessity of admission * Perform concurrent reviews to determine need for continued stay * Discharge planning assessment and coordination of delivery of services necessary to meet individual patient needs * Identify and address opportunities for improvement in patient care, including communication with all disciplines involved to coordinate effective in-patient stay, as well as safe and timely discharge * Patient advocate from admission through discharge * Participate in daily rounds and collaborate with a multidisciplinary team * Conduct and participate in daily care conferences driving length of stay. adults and pediatrics PCPs and developed individualized care plans
