Review and evaluation of medical, psychological, social and vocational information involved in the planning of realistic employment outcomes for individuals with disabilities. Analysis of the physical demands, specific skills and knowledge requirements, specific tasks and environmental conditions related to a particular job in order to assist with decisions regarding placement, reasonable accommodations, modifications, and rehabilitation technology. Establish, develop and maintain positive relationships with case managers, state, local and nonprofit entities, school transition coordinators, and diverse businesses toward the goal of effective job placement. Collaborate with consumers and service providers to develop appropriate employment goals and to coordinate services. Monitor job placement services and follow up to ensure successful employment. Teach job seeking skills courses and workshops (topics included finding job openings, resume writing, interview skills, disclosure of disability) to empower consumers in their job search. Provide on the job training and develop tools to assist consumer on the job.
