Senior Medical Social Worker with over 30 years of experience providing high end social work services such as coordination multidisciplinary teams to formulate plans for aging patients and those entering hospice care, as well as, evaluating patients needs at admissions and formulating appropriate treatment plans, providing on-going case support and advocacy services to the family, patients and friends related to their social, spiritual and emotional needs of patients.
