Case Manager
I would like to work in a behavioral healthcare facility that would allow me to access, plan refer, inform and link to other organizations that provide assistance to help individuals at whatever point they are in life. I would also like to develop and/or participate in treatment of these individuals. Of course, proper documentation including progress reports and summaries are necessary to avoid ineffective procedures that were tried in the past and to evaluate the individuals' progress for sustained treatment.

- Presently, I am in the Case Management Unit. I research information such as income that was reported to the state of Florida that may or may not be on the application for assistance (food stamps, medicaid or cash assistance). Prior to being on this unit, I was in the Processing Training Unit where I learned to process applications using and requesting doucments that would help me to determine eligibility for assistance.

- Duties: Retrieve information from the medical records, physicians laboratory and raidology reports about the presentation, diagnosis, treatment and follow-p of patients with cancer. This information was interpreterted, documented and coded according to standards set by the American College of Surgeons/Commission on Cancer and the Florida, Cancer Data System. Other duties case identification using the International Classification of Diseases (ICD-9-CM) and the International Classification of Diseases for Oncology (ICD-O), patient care evaluation studies as a result of physician request and annual chart review with physicians.

- Medical Records Supervisor: Supervised fourteen employes in Record Processing who retrieved and sent medical records to the unit a returning patient, complete birth certificates of all the new borns and send them to the helth department, collected all the records of patients who are discharged from the hospital, flagg charts for appropriate physician dictation and signatures and fulfill patient; medical & insurance requests for meidcal information. Cancer Registry Supervision included setting up cancer conferences and cancer committee meetings, keeping minutes of cancer program activities and cancer case presentation, submitting documentation for continuing education certificates for doctors and licensed individuals in attendance, evaluations studies showing that at least 90% of the annual cancer caseload is staged and reviewed by physicians. I was alos responsible for producing and distributing an annual report to the medical staff and the community.
