40 hrs/w Duties: * Coordinate spiritual support to patients, families, caregivers and hospice staff. * Implemented and coordinated all activities relating to the spiritual aspect of the hospice program and bereavement activities to include documenting patient records for progress and care plans. * Provided and coordinated spiritual support and palliative care to patients, families and caregivers through contacts with patient/family clergy or religious representative when appropriate. * Conducted funeral services when requested by the patient's family and assists with bereavement follow-up service as appropriate. * Participated in on-call counseling services when required and provide spiritual support to the hospice staff. Duties: * Served as a minister as diagnostician a member of the interdisciplinary team for health care and spiritual comfort and palliative care to patients and their families in a 900-bed facility. * Provided chaplain services responding to trauma and unit calls of patients to provide counsel and for patients their families and hospital staff. * Provided one-on-one counseling for crisis intervention family support spiritual support grief ministry pre-and post-surgical counseling religious sacramental ministry and staff counseling. * Performed regularly televised devotions via the hospital closed circuit system. Over 5000 one-on-one first-time visits to patients in five (5) assigned units conducted within eleven (11) months of my residency to include over 2500 follow-up visits of grief care and counseling to patients. Attended funerals and special seminars and worship services. Provided extensive work assistance to palliative care patients. (Administrative duties 25 percent and Pastoral Care functions 75 percent)
