Summary: * Pharmacist with extensive experience in hospital, retail, and ambulatory Care Clinic. * Discharged patients using face to face to communicate and make interventions. * Conduct medication reconciliation for inpatient and outpatient settings, and evaluate the domains involved in the transition of care process for newly discharged patients. * Collect patient specific information, and list important medication related problems which may require changing the course of therapy. * Provide bridge clinic for medication management and anti-coagulation services. * Knowledge of medications and their correct administration based on age of the patient and their clinical condition. * Dose and monitor TPN, warfarin, aminoglycoside, vancomycin, renal impairment, hepatic impairment, and hemodialysis patients. * Provide Medication Therapy Management: Ask the Pharmacist, at Desoto living Expo. * Participate and several foreign medical and non - medical mission trips. * Establish and maintain cooperative working relationships with Physicians, Nurses, Administrators, and other health care industry personnel. * Deliver the highest quality patient service in a spirit of love, empathy, assurance and confidence. * Provide Managerial functions by planning, organizing, staffing, leading and controlling. * Accomplish tasks effectively as a leader, and maintain relationship with co-workers. * Making decision from available alternatives by clearly identify the problem itself. * Effective in coaching, mentoring, and delegating tasks to staffs members. * Instill a sense of purpose in my co-workers, set high expectations, encourage autonomy, offer learning opportunities, communicate appreciation, and have fun. * Built effective teams by allowing full sharing of information, promoting respect for one another, creating non-competitive environment, focusing on solution, and feel a sense of belonging. * Excellent interpersonal skills and organization skills, good oral and writing communication and fluent in multiple languages. * Have an open mind, ability to work independently and as a team member, able to prioritize multiple job tasks and learn quickly. * Certified Clinical microbiologist, hematologist, chemistry, immunologist, Pharmacy/IV, APhA/MTM, APhA/immunizer, basic life support and ACLS. * Basic level skill in Microsoft word, Excel and Power point, Pharmacy software, like MEDEX, MEDITECH, ALLSCRIPTS, Rx CONNECT, PROTOUCH, and many more.
