Conduct facilitate and evaluate group at Northeast Florida Healthy Start Coalition Jacksonville, FL * Conduct needs assessment to determine priority group session topics. education sessions. * Create partnerships with agencies to facilitate group education sessions. Plan coordinate and execute community outreach programs. * Conduct outreach to recruit new members and to offer free services such as free health screenings. * Plan and implement participant incentive events. * Work to find community resources and put together community resource guide for case managers. * Organize and document community donations. * Promote Azalea Project to highlight the program.
