SUMMARY OF QUALIFICATIONS * 4 years of experience providing personal support care to seniors and youth in long term homes, rehabilitation care setting and hospitals * Completion of a Personal Support Worker program and received a certificate that meets the vocational standards established by the Ontario Community Support Association * Proven communication skills to document resident's progress in a computerized system and orally communicate with multi-disciplinary team * Demonstrate the ability to establish rapport with staff, families, patients and the general public to aid in the development of care plans * Professional communication skills include fluency in English * Currently uses mechanical lift to transfer, lift, and turn patients * Ability to organize work in a manner that ensures resident safety * Recent experience using basic infection control principles and the use of personal protective equipments to work with the patients * An updated Police Reference Check (PRC) with the ability to provide vulnerable sector screening and proof of immunization and tuberculosis screening
