As a clinical pharmacist coordinator my main objective would be to implement a number of clinical protocols designed by pharmacists, and run by pharmacists. A few examples woould be a consultant clinical role in nutrition, kinetics, coumadin therapy/Lovonox bridge, transplant clinical pharmacist. When any of these serices are warrentd the RN or MD would page pharmacy for a consult on a patient needing TPN nutrition. The pharmacist would go and evaluate the patient, find out if any of the gut is working, and evvaluate laboratory values. The combination of all this information, the pharmacist would make out a custom TPN order for that specific patient. The pharmacist would continue to see the patient daily, order CMP, Ca, Mg, Phos. The results of these blood tests would dictate if any changes need to be made in the formula. The kinetics ser
