Summary of Qualifications Clinical skills to include: * Assess, provide and document nursing care for designated patients in accordance with the physician's plan of care, agency policies and procedures. * Medication administration and management, wound assessment and treatment. * Treatment coordination, mental assessment, nutrition assessment and education. Implement and coordinate services provided in addition to utilizing agency and community resources to meet patients needs. * Counsel patients to participate in treatment plans. Communicate with physicians, case managers and other personnel regarding patient care. * Supervision and training of CNA's and HHA long term care, sub acute, residential and community settings.
