Daily Duties at Coram Healthcare Home Infusion :
• Process and evaluate physician orders, pharmacy dispensing, patient appointment, logistic coordination, and inventory management • Provide clinical information to doctors and nurses on different diseases such as diabetes, renal, cardiovascular, and cancer for intravenous, subcutaneous, and epidural injections • Review medication profiles and counsel healthcare professionals on insulin therapy usage, side effects, and storage • Participate in therapy management and customization tasks such as adjusting TPN & cancer treatments with dieticians and physicians • Evaluate abnormal lab results and recommend dose adjustments to ensure the safety and efficacy of Vancomycin and Aminoglycoside therapy • Review and update stabilities of drugs and maintain all Coram policies and procedures in accordance with the Board of Pharmacy
