Utilization management of cases through admission to discharge. Clinical reviews to payors for admission and continued stay approvals. Discharge planning assess, reassess, development of plan and execution of final plan with MD, ancillary services and family input. Documentation of initial CM assessment and discharge plan in chart and CM program. Communication to insurer for assistance with post discharge services and authorization needs to transition to next level of care. Quality issue and avoidable day documentation. Education of medical staff regarding CMS documentation requirements related to Attending certification for admission, Code 44 protocol, and Important Message for patient right to refute discharge, as well as Face to Face form to justify need for homecare services. Reinforcement of documentation for severity of illness and intensity of services while patient hospitalized. Assist MD staff with communication between insurance carriers to assist with denial overturns. Minimize LOS and excessive use of services.
