Daily Duties at Waterview Nursing Care Center:
The Care Plan Meeting Coordinator worked in conjunction with the MDS Coordinator. This required knowledge of the MDS evaluation process, the mandated scheduling for Care Planning dates consisting of the Initial Assessment, the 14 day Assessment, 30 day, 60 day and 90 day each following a specific calender of start and stop dates as well as submissions to the State which generated Medicare reimbursement based on categories determined by these assessments. The process was very specific and the scheduling of Care Plan meeting dates was the result of these events. There was also a category which determined "significant change in status" leading to another assessment window as well as another Care Plan date. Family members were invited to cyclic meetings as well as the patient themselves. For a Facility of this size, Care Plan meetings were generally conducted each Tuesday and Thursday and involved an approximated half a day event. Prior to meeting time, the Care Plan Meeting Coordinator is involved on the team of completing the MDS assessment booklet with each patient scheduled that week and the resulting electronic submission of data resulting from these assessments. Rehab services and Level of Care were driving forces to rank each patient on a reimbursement scale. Accuracy incompletion of these assessments was imperative as well as financially advantageous for the Facility. Any information entered in the Assessment had to be validated by actual documentation which could be readily tracked by Surveyors/ Auditors as needed.
