Daily Duties at Longview Wellness Center:

• Conducted the qualitative evaluation of a Federal grant through interviews and a focus group. • Created a system to monitor patient data monthly • Prepared first draft of the quarterly reports and continuation request • Recruited for and mediated over a focus group for patient input on the program • Completed a fidelity ssessment • Created a quality improvement plan for the clinic, including conducting a patient satisfaction survey • Conducted interviews of medical and grant staff • Created pre and post test for training (Gatekeeper and CME event) • Co-chaired a committee in the beginning phase of planning a mental health awareness event. • Assisted with locating speakers for a conference for medical provider Outreach Coordinator • Created a brochure promoting a mentoring program for males • Educated middle and high school students about responsibility • Promoted the Longview Wellness Center in the community • Prepared agenda and minutes for coalition meeting • Planned activities for children at a local recreation center • Participated on the planning committee for a health fair for National Women’s Health Week • Attended health fairs and distributed information. • Educated kindergarten, second, fourth and fifth graders on violence prevention • Planned activities for children at a local recreation center • Participated on the planning committee for a health fair for National Women’s Health Week • Attended health fairs and distributed information. • Educated kindergarten, second, fourth and fifth graders on violence prevention
