Daily Duties at Interim Health Care:

Licensed Practical Nurse Post-operative wound care, complicated medical management with ongoing rehabilitation, and recuperation of geriatric patients after acute hospitalization Services included limited to; medication setup/compliance assessment, diabetes management teaching, home safety evaluation, bowel and bladder care, cardio/pulmonary assessment (C/P) and home hospice. Assesses the patient's needs, identified appropriate services and created an individual plan. Served as the chief problem solver and communicated with other caregivers and the physician. 4/1998-12/2000 Interim Health Care CNA Personal care assistance which included bathing, dressing, grooming, medication reminders and limited restorative care. In-home respite for caregiver Education
