Daily Duties at Volunteers Of America:
2005-2006: Foster Case Manager- Carried a maximum caseload of 15 relative/traditional family cases. Provided case management for selected youth/families who come under Family Court jurisdiction. Conducted strength-based assessments, goal setting and develop client centered treatment/service plans. Lead in building effective treatment team to implement a community-based treatment plan with each client/family. Networked and coordinated, linkage of support services to meet the need of clients/families. Maintained a caseload of program participants, monitored participation for compliance, prepared and maintained case records. Wrote social histories; assisted with Orders of Protection. Worked to build a strong network in assigned community including schools, mental health/substance abuse and other providers. Wrote reports (SACWIS) per DCFS guidelines, attended and testified at court hearings as needed. Gathered information to assess needs of clients. Maintained relative foster care, traditional foster care, treatment, and adoption foster care caseload. Made necessary referrals and assisted clients to stabilize their lives. Responsibility included the removal and the reunification of children from various dangerous situations. Transported clients to appointments, home visits and court hearings as necessary. Performed simple to complex case management tasks.
