Daily Duties at Elwyn Community Residential Services:
Responsible for monitoring consumer progress through the Individual Support Plan (ISP, IPP, IWP, Monthly and Quarterly reports) while serving as the liaison for internal referrals to families and other community agencies. Responsible for leading an interdisciplinary team in the development and implementation of plans structured around individual’s goals and outcomes. Serve as an advocate for quality of life in a caring and hospitable environment for all Elwyn consumers. Supervise all members of the residential service staff while coordinating with Community Residential Service site supervisors. Fulfill all functions of a consumer advocate by all applicable Pennsylvania State regulations. Serve 11-14 individuals with developmental disabilities with a multitude of staff members at multiple locations which operate 24 hours a day, 365 days a year. Contacts: Funding agencies, families, guardians, advocates, providers, residential staff, representatives from community resource agencies, medical professionals, consultants, other individuals with disabilities, representatives of County Mental Retardation agencies and Elwyn staff.
