Daily Duties at The Plaza Nursing And Rehabilitation Center, Bronx Ny –pac Leaders Company :

• Manages and balances time in providing care for the patient and documentation in the health electronic record to ensure the facility get reimbursed and remain financially stable. • Provides primary care for sub-acute and geriatric patient in collaboration with the attending physicians • Attends to emergency situations • Initial comprehensive assessment on admission • Makes rounds and sees residents who require visits on regular basis, upon request and or need immediate attention. • Order lab test and reviews laboratory and diagnostic results • Make appropriate diagnosis, treatments and referrals where necessary. • When clinically appropriate, sees residents as part of the evaluation and follow up of abnormal labs or test results. • Appropriate documentations in the electronic health record system. • Reviews, signs and dates consultant notes • Reviews and responds to items in the communication books (Rounds book) • Collaborates with the physician and other health care professionals to insure optimal delivery of resident care. • Available for resident/ family discussions, conferences and comprehensive care meetings • Writes prescriptions as per New York State Law. • Orders consultations and make appropriate referrals to other health care professionals including physicians, dieticians, social workers, physical therapists, occupational therapists, speech therapists, nurses, activities/recreational therapists. • Completes discharge summaries. • Performs medical clearance for uncomplicated dental procedures. • Orders labs for medical clearance. • Communicates professionally and courteously with residents and or family members regarding resident’s condition, plan of care and change in status. • Maintains professional appearance and attitude in carrying out job duties—Respects cultural needs and values of residents, families and staff. • Completion and signing of death certificates.
