Facilitated patient transition and Case Management for an average of 275 plus patients a month through constant assessments and following patients progress in Acute Care Settings in the Hospital. Ensured patient safety through assessment, planning, implementation, and evaluation. Developed transition plans based on clinical guidelines and protocols to assure quality and efficiency. Adhered to post-acute plans and directed completion of services. Nurtured relationships with health systems and providers. Executed quality assurance of transitional care and continued to meet or exceed goals.
