Integrated strengths in the areas of communication, interpersonal skills, healthcare leadership, and patient care experience to case manage a panel of 75 individuals identified as chronically ill, elderly, stroke, and/or debilitated patients with an emphasis on appropriate coordination and provision of post-discharge care to minimize the risk of hospital readmission. Cultivated strong relations with patients and their families as well as multidisciplinary providers to provide guidance and instruction on treatment plan compliance to empower individuals to achieve maximized recovery gains. Liaised between patients, Case Managers, and Medical Directors to assist with utilization reviews ensuring appropriate patient status placement and provision of services that met identified care needs.
