Daily Duties at Metro Plus Heath Plan:
• Coordinated and organized care for MetroPlus insurance holders. • Referred members to managed care services and other available services as needed. • Provided on-site authorizations for any follow-up services from an outpatient visit or inpatient stay (e.g., home care services, DME, out-of-network referral). • Assisted with admission and discharge planning. • Prepared presentations and provided health education to members using information from content experts and peer-reviewed literature. • Attend meetings of cross-Departmental/Office project teams and maintained work places for cross- Departmental/Office projects; briefed the Regional Manager at MetroPlus on project status. • Collaborated with project planning and implementation. - supervised 2 customer services representatives
