WESTCHESTER MEDICAL CENTER ∙ Valhalla, NY Hospital Pharmacist (2009-2015) Responsible for providing centralized services and support to a multidisciplinary clinical team. Collaborated with healthcare providers to ensure accurate prescription records, solving clinical and therapeutic drug application issues. Evaluated and Siemens provider verified orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interaction significance, frequency, allergy route, rate of administration, formulary status and redundancy. Prepared and dispensed outpatient prescriptions, clarified information or discrepancies with prescribers, caregivers for patients for completeness , accuracy and safety. Provided medication use information and precaution to either patient or care giver. Interpreted, clarified, rectified medication and IV orders including medication reconciliation for inpatient and outpatient. Clarified orders through direct and timely contact with prescribers or designated practitioners. Documented encounters in Siemens clinical interventions. Utilized automated medication dispensing cabinets as needed for urgent medication needs. Demonstrated knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. Assured safe and appropriate use of medications and advocated for rational drug therapy. Monitored all patients on specified target medications and initiate pharmacokinetic dosing protocol. Manage pharmacy operations and pharmaceutical care for all patient age groups including outpatient service per assigned area. Provided patient-specific pharmacotherapy recommendations and drug information to clinicians. Implemented all drug use policies and formulary decision of Pharmacy and Therapeutic Committee. This included clinical management activities such as controlling use of high cost drugs, formulary restrictions, high used drugs, therapeutic interchange, switching IV to PO to optimize cost effective drug usage , enhance patient safety while maximizing clinical outcomes. Identified, monitored, and reported adverse drug events and medication errors according to established procedures and regulatory requirements. Performed nursing unit inspections. Assured proper maintenance and storage of medications. Corrected and reported discrepancies. Dispensed, maintained records for inventories of controlled substances, investigational drugs according to hospital policies and procedures. Filled all pixies stations in the operating room, rotated the stocks, resolved and documented all controlled substance discrepancies. Oversighted workflow operations, checked all pyxis restock, oral and IV medication compounding batch production, Filled, checked and approved all emergency drug boxes with medication approved for ACLS resuscitation in pediatrics, neonatal and adult patients in various areas of the hospital. Filled and checked medication carts. Supervised and trained pharmacy technicians responsible for IV medication preparation and delivery. Prepared and dispensed all medications that required preparation in aseptic environment. Managed all IV equipment including daily calibration-corrections, daily temperature documentation-interventions. IV admixtures with additives included chemotherapies, large volume solutions, ophthalmic drops, intrathecal, irrigations, globulins, specialty products for clinics, allergen solutions for outpatient. Compounded all other parenterals required for patients per policies and procedures. Accurately and successfully, processed and transmitted via computer to Braun Mcgraw’s CAPS over one hundred (100) for parenteral nutrition solutions daily to be prepared for 24 hour treatment . Screened and Verified all orders for appropriate dosing, compatibilities, route, rate, frequency, redundancy, formulary status and duration per hospital policy and procedure. Provided oversight to four technicians per shift plus helped train and supervise new pharmacist and pharmacy interns. Also relieved Night shift daily, managed all incoming calls and services with the help of two technicians. Responsible for Providing Pharmaceutical care to patients within service groups at the university hospital including inpatient, outpatient, Behavioral Health Center,Taylor Care Center, Maria Fareri Children’s hospital. Managed and prioritized processing of orders which included 200-300 orders at any given time. Provided services to six major groups. The largest service center comprised General Medicine & Surgery, Surgical Oncology, Oncology, Second service center provided services to Maria Fareri Children's Hospital(Newborn Nurseries, Neonatal and pediatric ICUs, Pediatrics, Special care, Well baby nursery Third service center included services for Neurology ICU , Med Surg, AIDS, Transplant, Vascular, Reab Medicine, Orthopedic/stroke Fourth service group included Intensive care units(Medical, Cardiothoracic, Coronary, Surgical center). Burn center, Bariatric care, post-operative care, post-anesthesia, emergency(E.R.) labor & delivery, Observation, Prison/jail patients. Taylor Care Center , services included skilled nursing, sub-acute, respiratory care, rehab and healthcare patients Behavioral health pharmacy service included psychiatric care for all patients age groups. ∙
