Daily Duties at Humana :
Provided Care/Case management support to Senior Citizens living independently in the Raleigh, Durham and surrounding areas. Ensured that seniors recently discharged for hospitalization back into their homes or assistant living center were connected to all health and community services and available to them through Humana and other contributing agencies.
