Daily Duties at Americare Home Health:
Manages and directs a team of Nurse Case Managers, Therapist, Medical Social Workers and Home Health Aides ensuring safe, effective and appropriate home care services. Collaborates with Agency Director in establishing operation budget needs and priorities for the department. Provide administrative support in monitoring clinical services and staff development. Ensures accuracy, completeness and timeless of clinical documentation in accordance with agency policies and procedures, regulatory requirements and industry standards. Educate patients and families on medications, conditions and services supplied under HIPPA, Medicare guidelines and agency policies. Provide leadership to team and support staff in weekly case conferencing and performance review. Review OASIS data collection to assure accuracy and consistency with requirements. Communicate with clinicians and provide training as indicated to achieve compliance. Follow agency guidelines for disciplinary actions in accordance to Human Resource policies and legal guidelines. Perform other duties as assigned: Marketing, Community presentations, Care coordination for the adult mentally challeged/ psychiatric patients
