Daily Duties at Genesis Health System:

Meet with patients who have been admitted to the hospital due to an illness or surgery to establish a rapport with them and their support systems. It is important to learn about their lives outside of the hospital to determine their needs when they are going home. Discuss options for services that might be needed and how insurance coverage pays for services. Discuss and provide information on skilled nursing facilities if the patient is not going to be able to return directly to home at discharge but needs physical/occupational therapy before being able to return home. Care coordination can vary among patient to include the patient does not have any needs when they go home to their health has changed and they now want hospice services. Care coordination must know the resources that are available to the patients, cost that are involved and must be able to set up the services for the patient. Part of a multidisciplinary team that works together with the patient and their family to include, plan and provide the best possible to the patient during hospitalization and after.
