Daily Duties at United Health Care/Monarch:
Handled 20-40 patients telephonically discussing progress and modifying care plans for the best possible. Provide reminders for appt's, would arrange appointment and discuss care needs with the physicians involved for consistency and continuity of care. I would also go out to Fountain Valley Regional Hospital 3+ times a week to see the patients that were admitted and worked closely with the hospital discharge planner in regards to network providers and authorizations so there was no delay in discharge as well as reducing the risk of re-admissions.
