o Information Security Management Process design and Strategy; o Implement of governance policies and procedures to prevent, detect, contain, and correct security violations to include exposure of personal identifying information, PII; o Conducted accurate and thorough assessment of the potential risks and vulnerabilities to the confidentiality, integrity, and availability of electronic protected health information; o Implemented procedures to facilitate regularly review records of information system activity, such as audit logs, access reports, and security incident tracking reports. o Implemented security measures sufficient to reduce risks and vulnerabilities to a reasonable and appropriate level to comply with Section 164.306(a); and, o Developed and applied electronic sanctions that mitigated user failure to comply with the security policies and procedures.
