• Supervised all Case Management staff by providing training, education and guidance • Conducted performance appraisals, staffing plans, schedules and make hiring decisions • Worked closely with all Physicians, Service Providers, Vendors and N.Y.S Department of Health to ensure requests, care and safety is provided • Managed and preserved the residents’ routine and independence by coordinating services (i.e. P.T., psychiatric services, Paratransit) • Provided crisis intervention and support as needed
