• Assisted clients in attaining Independent Living Plans, Permanent Housing. • Acted as liaison for Department of Social Services. • Performed intakes, psychosocial assessments, interviews, and bi-weekly documentation. • Worked with and coordinated with other case managers, van operators, and administration. • Conducted Mental Health, Disability, Employment and Housing Referrals. • Networked and worked in coordination with other agencies. • Supervised a staff of 5 • Trained new professional staff members • Performed many other duties as requested by the administration.
