Daily Duties at Well Sense Health Plan:
Telephonic and face-to-face care management with members who have complex medical health challenges. Members are pediatric and adult. Assess, plan, implement health care goals with the member via a care plan; support member with all possible resources to overcome barriers in achieving their health care goals successfully. Reassess current approaches that are not producing results and/or development of new barriers.
