Educated patients, their families and facility staff regarding all applicable services and products provided by Walgreens Infusion and performs on-site pre-discharge education on various infusion and enteral therapies.  Maintained positive relationships with various referral sources and serves/ acts as liaison between the hospital and home infusion setting.  Interacted with area service providers on a daily basis to conduct on-site evaluation of patients selected by the hospital for home infusion services and who have been referred to Walgreens Infusion.  Effectively communicates with Walgreens account managers/ sales and participates in development and presentation of home infusion educational programs, in an effort to promote optimal outcomes and grow market share.
