Daily Duties at Patient Pathway Da Vita Village Health :
Implementing and coordinating the development of Renal Case Management and discharge planning programs. Facilitates communication channels between Renal Case Manager and hospital patient care departments. Facilitates positive change utilizing principles of change and organizational theory. Programs are implemented with hospitals (UCLA, and Cedars-Sinai) across the country in a variety of markets.
