Daily Duties at People Advocating Transitions:

Maintain a relationship with targeted youth assisting them in classroom and behavior excellence. • Communicating with peers making sure all guidelines are followed correctly, assessments completed. • Communicate verbally and documenting the transition the targeted youth are making to excel. • Creating behavior plans an academic plan for targeted youth to maintain positive relationship. • Working along with my team to make sure all documents are worked correctly. • Following up on behavioral plans on a bi weekly progress make sure our targeted youth are improving.
