Worked with Social worker and physician and family closely. Arranged with home health to provided services, durable medical equipment companies, transportation companies, etc. to provide the patient and family the best possible care and assistance. Also would continue to monitor care with routine home visits, completing all forms and working with physician to gather forms for state approvals. Hires and supervise other team of nurses.
