Daily Duties at Personal Involvement Center:
Guided the needs assessment, development, implementation, and evaluation phases of individual and family plans of care (POC). Worked the 24-hour on-call line providing crisis management to families. Conducted child and family team meetings to improve service delivery and quality of individualized child and family plans of care. Maintained positive relationships with families, service providers, in addition to community and governmental agencies.
