Developed person centered plans for individuals with developmental disabilities. Completed Plan of cares and justified services for individuals. Monitor the service that was being provided including fiscal management. Completed the developmental disabilities profiles for each individual. Provided linkage to services for the individuals in the community. Completed an individual support plan and monitored to progress on the goals and supports given.
