Daily Duties at At Home Health Care:
As Community and State case manager I worked closely with the Department of Aging and Disability to ensure that area patients who qualified for the program received the services they were entitled to. These services included attendant services, DME equipment, medical supplies, home modifications, and services such as MOW and the care of a physician to meet their needs.
