Daily Duties at Arbor Hospice:
Worked with families to help them select end-of-life care. Planned and executed end-of-life education for physicians, nurses, fellows, and social workers. Worked closely with hospital discharge planners, physicians, palliative team members, case managers, social workers, management, inpatient unit, and education staff to generate referrals. Acted as liaison between hospitals, physicians, patients, and the company to provide information and maintain positive relationships.
