Assess patients knowledge base of his situation, what needs, physical and menta care is required to prevent increasing complications from dz processes; perform Oasis SOC, provide education on dz processes and how to maintain those dz processes, future phy appointments, setting up goals and time frames for achieving goals; performing ROC, Discharges, consulting with Physician and nurses involved with that client to achieve goals.
