Daily Duties at I Am Unique Special Care & Case Mgnt:
Bring emotional support to children with special needs and their families by engaging children with special needs to live their lives fully to the best of their abilities through support and facilitation of patients’ developmental needs, as well as family well being.Design preventive measures,prevent infections,and reduce frequency of recurrent infections.Reduce hospitalization by 80% compare to when children were institutionalized.Successfully maintained and coordinated communication with the agency to ensure that all patients’ needs are met. Prevent deterioration in children with special needs conditions through activities to maintain their proficiency, and through patient and family education. Find and maintain productive ways to improve patient's health status in collaboration with a multidisciplinary health team.
