Assist in setting up data collection and tracking system. Responsibilities included: • Secondary data extraction from web-based sources and its compilation. • Update questions to be included in Community Health Survey tool for valuable data collection. The tool is currently used by Carilion Clinics to perform 2015 Community Health Needs Assessment as required by the IRS. • Propose evidence based measures and indicators for established expected outcomes and streamline with partners’ current primary and secondary data collection practices • Prepare documentation and report findings to team for further decision making.
