 Nurse recruited to serve as a diabetic health coach for patients with diabetes.  Provide thorough assessments on new patients to determine clinical risk and readiness to change, and then design programs specific to each client.  Maintain accurate and timely documentation in patient management software of telephonic communications. Prove ability to create trusting relationships via phone that lead to open communication and healthy discussions about diabetes
